If we haven't met yet, completing the screening form below is strictly for my eyes alone and enables me to screen you for my safety. Except for your contact information, none of your details will be kept on file, hopefully allowing us to get back in touch shortly. 
I require government ID/passport verification from every new client, or a paid videocall on SP, for my own safety and peace of mind.

I require a non-refundable 20% deposit in order to guard against cancellations and no-shows. Deposits are only refundable if I have to cancel. If I cancel you will get your deposit back within 48hrs. However, I will apply your deposit to a new date within one week of the originally scheduled time if more than 48 hours notice is given.
